RX Template

To customize an orthoses & save the selections on the Performance condensed RX form; do the following:

1. Download the PDF below & save to the desktop of your PC.
2. After the writable PDF is saved to your desktop, open to make your selections.
3. Once open, in the upper right corner, in the space provided, name the orthoses.
4. Make selections to customize the orthoses to save & reprint at a later time. Leave patient information blank if choosing to reprint this form as a template.
5. Once the selections are made, right click on the form & “Save as” and rename the PDF with the orthoses name
